Once you have printed out the form, please fill it out and bring it with you to
your first appointment. Or if you like you can call the office to set up the fax
machine so that you can fax it in ahead of your appointment. This will save you
time in the waiting room filling it out during your appointment. Other wise you
can just bring the New Patient Information Form to our office when you come in
for your first appointment.

Please note: when you get to the part of the intake form dealing with symptoms,
please be as complete as you can and identify symptoms that you are having
now or recently had. Oriental medicine bases its diagnoses on this information
and some other things we will do on your first appointment. Don't leave things
out like you might with an MD as this will compromise my ability to diagnose and
treat you properly. Remember you have nothing to fear; we don't use drugs or
surgery in our treatments!

Also be sure to list all pharmaceutical medicines you are taking as these could
be producing some of your symptoms. Additionally let me know if you have any
electronic devices like a pacemaker or any metal in your body.
